19 - Request Collection Kits
To request specimen collection kits, site users should submit a kit request through BioMS. Below are the step-by-step instructions for placing a Kit Request.
Step 1: In the left navigation bar of BioMS, click on Kit Requests under the TASKS section.
Step-2: The Specimen Kit Dashboard will be displayed as illustrated below. To initiate a new request for a kit, click on the "Place New Kit Request" button located in the top left corner of the page.
Step-3: The CREATE NEW SPECIMEN KIT REQUEST page will appear as illustrated below. All mandatory fields are marked with a red asterisk (*). For specific studies, the Patient ID may also be necessary, and this requirement will be emphasized as mandatory once a study and its corresponding kit are chosen.
If the Requester selects Regular Shipment, the Estimated Shipping Date (i.e. date Repository tenders the shipment to FedEx) will be within 10 Business Days from the current date. The actual delivery date at the requester’s site will depend on the standard transit time for the FedEx Express Saver service.
IIf the requester needs kits to arrive sooner, they can choose Expedited via FedEx. While the Repository cannot guarantee next-day delivery, requests submitted before 12 Noon Central Time are likely to arrive the next business day, and no later than two business days from the date of the request. In this case, the Requester’s FedEx account number must be provided. Please note that the FedEx Shipping Account number is a nine-digit number, and a valid account number must be entered without any dashes or spaces. The actual delivery date at the requester’s location will depend on the standard transit time for the FedEx Priority Overnight service.
If the requester selects Expedited via non FedEx(Requester provides shipping label), then the requester will have to email the Shipping Label(s) to the respective repository. The email address is shown on the top of this page once a Study Kit is selected.
