 BioMS Specimen Logging Form

This form is to be used whenever you cannot access the BioMS application or are unable to log your patient's specimens in the application. Please download and print the form. Fill out the required fields, include a copy with the specimen shipment and email a copy of the completed form to the help desk at BioMShelp@email.wustl.edu .
